The injured worker must report an injury to a manager and Human Resources within 30 days of the date the injury occurred.

- Take the Employee Packet – Required Notices with you to the visit
- Give a copy of any doctor’s note to Human Resources

Information for Managers
The manager must report an injury to Human Resources within 8 days of receiving notice of the injury.

- Use the Supervisor’s Packet to get detailed instructions for a workers comp claim
- Complete the DWC-001, Employer Report of Injury or Illness and send to HR
- Request the Employee Report of Injury from the Employee and send to HR
- Give the employee the Employee Packet – Required Notices to the employee
- Encourage the employee to use an in-network provider by using the Link to Alliance Website (Provider Finder)
